远隔缺血后适应与基因差异表达的研究进展

摘 要:急性脑梗死严重威胁人类的健康,实施早期有效的溶栓治疗、恢复缺血脑组织的血流和功能是临床公认有效的治疗手段。但溶栓治疗有严格的时间窗限制,能在早期行溶栓治疗的患者仅有5%,且并发症较多,为了攻克这一难题,缺血后适应对缺血脑组织的保护作用逐渐进入了人们的视线。目前远隔缺血后适应(RIPC)已成为当今研究的热点,通过对非致死性器官如远端肢体进行短暂非致死性重复缺血-再灌注处理,可明显减轻已缺血器官的缺血-再灌注损伤,对于已缺血脑组织可减轻其炎症反应,改善脑水肿,减小梗死体积。对于这一结论,许多研究都已证实,然而其发生机制尚不明确,目前较为公认的假说有抗细胞凋亡、抗氧化、抑制炎症反应、抑制一氧化氮(NO)释放、增加细胞因子等。本文将就基因的差异性表达作一相关性探讨。
